Priority Areas of Focus
The greatest overarching interest area is to address the health challenges experienced by those who are marginalized and with greatest socio-economic need.
Priority Populations:
Older Residents (65+), Low-Income, and Marginalized Racial/Ethnic Minority Groups
Three Priority Long-Term Strategies:
TRANSPORATION- Free Intra Town Shuttle Bus to increase access to food markets, health services, town activities, social engagement, and public transportation
DATA DASHBOARD- To track and monitor health equity and wellbeing indicators across town departments.
AGE FRIENDLY MILTON- To plan for a community that accommodates people of all ages, especially older people.
Methods
The Partnership for a Health Milton (PHM) will operate as an independent board of directors, collaborating with a variety of town departments and/or regional organizations. It will focus its efforts on:
-Being an advocacy and advisory group for promoting community health in Milton
-Raising community awareness and resources to support public health
-Sponsoring “special projects” and/or grant-making to worthy community initiatives aligned with PHM’s mission to promote Milton’s health and wellbeing
-Focusing on Policies, Systems and Environmental Change to improve the public health delivery system and social determinants of health
PHM will not engage in being a service provider organization.
Approach
PHM will actively include the voices and participation of community members through:
-Partnerships with other sectors/groups/institutions/committees in town and/or regionally to collaboratively accomplish projects and priorities
-Honoring the input and preferences of community members
-Using “Adaptive Leadership”, whereby PHM will lead where needed, and follow where it can support others. PHM will identify issues and mobilize resources that fit our mission and focus on issues that promote equity and sustainability.
Operations
Board members actively participate in the work needed to accomplish its priorities.
When appropriate, Board members conduct work by establishing Working Groups that include community members and foster partnerships with other groups in the town and in the region.
To achieve its priorities, PHM will engage consultants, students, and community volunteers as needed, and to support that work, PHM raises funds through writing grants, requesting donations, and by other means.
